We present the case of a 57-year-old man with emphysematous cystitis. The patient presented with a complaint of several days of lower abdominal pain and gross hematuria. Work-up included an abdominal CT scan that revealed gas within the urinary bladder wall, and cystoscopy confirmed diffuse submucosal emphysema throughout the bladder. Emphysematous cystitis is an exceedingly rare condition in the pediatric population, with this report representing the second case within the literature.

Emphysematous cystitis is a rare type of infection of the bladder wall by gas-forming bacteria or fungi. The most frequent offending organism is E. coli. Other gram negative bacteria, including Klebsiella and Proteus are also commonly isolated. Fungi, such as Candida, have also been reported as causative organisms.
